How to make a referral to The Oakleaf Group

The Oakleaf Group actively encourage referrers and prospective clients to visit the various services available, which can be invaluable when considering a rehabilitation package of care.

 

Referrals are welcome nationally from healthcare professionals and family members who in the first instance are invited to complete the Referral Form which is available on request.  All referrals are considered and if appropriate a free of charge pre-admission assessment will be offered either in situ or at any one of Oakleaf's facilities in Northampton or Nottinghamshire.  Oakleaf will aim to undertake a pre-admission assessment within 2 weeks of the referral being made.  The facilities and professionals included allow Oakleaf to respond to emergency situations as required.

 

Pre-admission assessments will essentially be the opportunity for Oakleaf's Nurse Assessor to gather all relevant information to present to the multi-disciplinary team for consideration of suitability, the outcome of which will be forwarded by a comprehensive written report.

 

Admission to Oakleaf Care will be agreed once funding is in place and arrangements will be made to ensure a smooth transition with as little disruption as possible, after which clients are encouraged to familiarise themselves with their new environment.  New clients and their families will be provided with a Service Users Guide, which provides useful information and is designed to answer any initial questions.
