Working with purchasers of care, Oakleaf are able to develop dynamic programmes which optimise each resident's potential.

Hilltop House (9 beds) - Northampton
Typically residents are admitted to Hilltop House and benefit from intensive individual rehabilitation programmes which determine short and long term goals whilst undergoing assessments in every aspect of their therapeutic programme. Sessions are purposeful and based around the individual, which include daily living skills through to supported community access, work-based skills and leisure activities.
Due to the highly skilled muti-disciplinary team Hilltop House are able to place individuals who may be physically dependent as a result of their injury in addition to cognitive difficulties.

Hilltop House Flats (4) - Northampton
Designed with independence in mind Hilltop House flats are typically the next step for residents who although require ongoing support, have shown improvements in their abilities to live more independently. Residents will have 'their own front door' and increased expectations in terms of responsibility and 'ownership' of their rehabilitation.
With continued support from the multi-disciplinary team residents are able to experience a greater sense of freedom with increased confidence and achievement through community access and possible voluntary work experience.

Hilltop Lodge (10 beds) Northampton
It is recognised that some residents benefit from a slower pace of rehabilitation or may require ongoing higher levels of support for an extended period. Hilltop Lodge is a purpose built facility designed to meet the needs of residents requiring longer term placements and support whilst engaging in their individual programme of rehabilitation.

Hunsbury House & Weston Favell House (8 community assessment placements) - Northampton
In conjunction with established services at Hilltop House, Hunsbury House and Weston Favell House are facilities which are designed to specifically assess residents placed in a supported environment with increased exposure into the community by adopting appropriate risk management strategies. Hunsbury House & Weston Favell House are suitable for those who are ready to move on from residential care so that full assessment of their ability to live more independently can be achieved. All residents will have continued access to a comprehensive rehabilitation programme which will focus largely on community reintegration and access to specific therapies available at Hilltop House i.e. horticulture to allow successful transition to independent or supported living with minimal risk of failure.

The Martins (10 beds) - Newark, Nottinghamshire
Similar to services offered at Hilltop House, The Martins offer brain injury rehabilitation placements to gentlemen with an acquired brain injury who would benefit from intensive individual rehabilitation programmes which determine short and long term goals and focus on maximising ones independence and ensuring a high quality of life.
