The final stage of the rehabilitation pathway is continuing community assessment.  The Oakleaf Group have developed several smaller services based in community settings to reflect the feeling of re-integration.  Community rehabilitation placements allow further assessment of long term needs to help identify the most suitable placement and support options that will reduce the risk of clients experiencing the 'revolving door' scenario of rehabilitation.

18 community rehabilitation placements have a number of purposes:  community integration, longer term community placement and all will benefit from seamless, comprehensive programmes of activities and specific therapies at Hilltop House i.e. horticulture, to ensure that gains made thus far are developed and transferable into every day activities of daily living.
